Yes you can actually! You can tweak any existing model to suit your needs! If you found a model you like and you want to add a hat, slap a hat on it! I do it all the time. However, most of my OC's are private and no one but super close friends irl see them. I don't make them public for reasons (because some modeler rules are dumb). But if you go public with you models, make sure you follow their rules. And if you have any more questions, drop me an ask or two~ ^^
What about changing genders or facial shape edits? I think that can be done in Blender. One more thing, I’m more knowledgable with Sculptris than Blender. Can models be edited in Sculptris?
I doubt I’ll go public with my edits unless I make scenes with them.

MMD isn't terribly hard to use. It all depends: do you want to make your model from scratch using blender or meta (which I honestly cant help you with) or would you prefer a handicap and use naked bases made by other users and then go from there (like adding clothes etc)? Additionally, you CAN edit someone's model and call it your "OC" but... it depends on their rules. And yes, we have those people who demand credit for ripped models -.- But we also have the "OCs" who are recolored Miku models
So even MMD isn’t safe from the crazies. I’m curious whether or not you can tweak existing models to look like your OCs instead of recoloring existing ones. I’ll have to look into this, thanks!
You know, I've always wondered if a melanistic or barbary lion would be a cool LK OC. I'm not super into the fandom so idk what the boundaries are between realism and semi-realism, but omg I'd love to see an OC like one of these done really well~
